APPLICATION CHECKLIST

Submit all of the following components in order to be considered for the NHLI 2009 program.

Click here to download the 2009 application form. Please note that the two recommendation forms are included in the application form.

• Recommendation Forms (2 required)
  (a) your immediate supervisor
  (b) someone you know professionally who is employed outside of your organization.
• A $25.00 application and processing fee must be attached to the application. Please make your check payable to NHLI.
• A letter indicating your employer’s commitment to release you from work to attend the four sessions.
• An original application plus e-mail six copies of all the forms to the address below.
