Please submit all of the following components in order to be considered for the NHLI 2008 program.

  1. Recommendation Form (2 copies are included in the application form)
    (a) your immediate supervisor
    (b) someone you know professionally who is employed outside of your organization.

  2. A $25.00 application and processing fee must be attached to the application. Please make your check payable to NHLI.

  3. A letter indicating your employer's commitment to release you from work to attend the four sessions.

  4. An original application plus six copies.
